Terms of Service

This page summarises the main terms for using PharmacyIQ. It is a readable summary, not the contract: the full Platform Agreement (version 2026-09-30) and your signed fee schedule govern, and win if anything here differs.

1. Who may use PharmacyIQ

PharmacyIQ is for licensed clinics and prescribers in the United States and the staff they authorise. You must be 18 or older to have an account, and every account is verified and approved before it can order. PharmacyIQ is not for patients or consumers: patients never sign up or pay here. You accept these terms for your clinic when you create an account or tick the agreement box at sign-in (the box is never ticked for you, and we record the version you accepted and when).

2. Accounts and security

  • Keep your details, licences, NPI and DEA registration accurate and current.
  • Keep your password private, use two-step sign-in where we require it (administrators and prescribers), and do not share logins.
  • Clinic owners are responsible for who they invite and for removing access when someone leaves. Tell us promptly at hello@pharmacyiq.co if you think an account is compromised.

3. Clinical responsibility

All clinical decisions stay with the prescriber: whether to treat, what to prescribe, the dose and the directions. PharmacyIQ does not practise medicine or pharmacy and does not direct any clinical decision. Compounded preparations are not FDA approved.

4. Orders and our pharmacy network

Prescriber-signed orders are fulfilled through our pharmacy network: each prescription goes to a licensed pharmacy in our network that is permitted to fill it for the patient’s state, which compounds, dispenses and ships it. The dispensing pharmacy is named on every shipment. PharmacyIQ does not compound, dispense or take title to any medication, and availability and fulfilment rest with the dispensing pharmacy.

5. Fees and payment

  • One charge per order. On the standard setup, each order is a single charge to your clinic: the medications at your clinic’s catalog price, plus one fixed PharmacyIQ platform fee, each shown as its own line on the order’s review step before you place it. The charge is made a few minutes after you place the order, to the card or bank account your clinic keeps on file, and the order goes to the pharmacy once the payment is on its way.
  • The platform fee is one fixed amount per order from your clinic’s own fee schedule: set in advance by service tier and, where your schedule lists one, as a set amount for a specific product. When an order mixes products, the single highest amount is charged once, and the fee is always shown before you place the order. It is never a percentage of, or based on, the price or quantity of what you order. Cancelled and rejected orders carry no platform fee.
  • Other setups. Some accounts are set up so medications are billed separately and PharmacyIQ charges only its platform fee, per order or on a twice-monthly or monthly statement. A few older accounts have a flat monthly plan instead of a per-order fee. Your Billing page shows which applies to you.
  • Fee changes are posted in the platform with written notice, apply only to orders placed afterwards, and never change an order you already placed. There are no hidden fees: nothing is charged that was not shown to you first.

6. Cancelling and refunds

You can cancel online, in the app, at any time, with no call or email needed:

  • Orders. A draft order is never charged. An order cancelled within the few minutes before its charge is made is never charged at all. Once an order has gone to a pharmacy and compounding has started, it may not be cancellable and remains billable.
  • Autoship refills. Pause or end any schedule under Orders → Autoship. No further refills are created or charged.
  • Flat monthly plan. It renews every month until you cancel, and the renewal terms are shown on your Billing page before each charge. Cancel with the “Cancel plan” button in Billing: the plan ends that day and no further monthly fee is charged. Per-order accounts have no subscription, so nothing renews.
  • Auto-pay. Turn it off at any time in Billing.

7. Patient information and HIPAA

Your clinic is the HIPAA covered entity for its patients’ information, and PharmacyIQ acts as your clinic’s business associate under a Business Associate Agreement (BAA). We use and disclose patient information only to provide the service to your clinic, as the BAA and the law allow. You are responsible for obtaining the patient consents your practice requires and for entering only the patient information an order needs. 8. Acceptable use

  • Use PharmacyIQ only for lawful clinical ordering, with a valid patient relationship and prescription.
  • Do not divert, resell or redistribute medication or office stock.
  • Do not misstate a patient, prescriber or clinical need.
  • Do not share logins or get around the platform’s access controls or security.
  • Do not upload malware, or probe, overload or disrupt the platform.
  • Do not export patient information into channels the platform does not provide for it.

9. Our platform, design and content

The PharmacyIQ platform, including its software, design, layout, look and feel, workflows, text, graphics, catalog presentation, documents and other content, belongs to PharmacyIQ and its licensors and is protected by copyright, trademark and other laws. We give your clinic a limited, non-transferable right to use it for its own clinical ordering while your account is in good standing. You may not, and may not let anyone else:

  • copy, reproduce, republish, frame or distribute any part of the platform, its design or its content, except the records of your own clinic and patients;
  • scrape, crawl or harvest the platform or its content, or access it with bots or other automated tools;
  • reverse-engineer, decompile, disassemble or try to derive the source code, structure or workings of the platform, except where the law expressly allows it despite this term;
  • use the platform, its design or its content to build, train or improve a competing product, service or dataset, or imitate its look, workflows or content; or
  • remove or change any proprietary, confidentiality or copyright notice.

Your clinic keeps its own data. We may use feedback you give us to improve the platform without owing you anything for it.

10. Emails and messages

By using PharmacyIQ you agree to receive the service emails your account needs: invitations, sign-in and security codes, order updates and billing notices. These are not marketing and cannot be turned off while you have an account. If we ever email you about our own services, you can unsubscribe with one click, and patient notification emails and texts that your clinic turns on carry their own unsubscribe and STOP options.

11. Suspension and closing an account

We may suspend or close an account that is inaccurate, misused, unpaid or out of compliance. You can delete your own login, and a clinic owner can close the clinic’s account, online in Settings → Security. Outstanding invoices remain due after closure. What we keep afterwards, why, and for how long (medical and prescription records must be kept for years by law) is set out in our Privacy Policy.

12. Disclaimers and liability

The platform is provided “as is”. To the extent the law allows, PharmacyIQ is not liable for indirect or consequential losses, and its total liability is capped as set out in the Platform Agreement. PharmacyIQ is not responsible for the clinical appropriateness, quality or fulfilment of any medication, which rest with the prescriber and the dispensing pharmacy. Nothing here limits liability that the law does not allow to be limited.

13. Changes to these terms

When we make a material change we update the version and date, tell clinic users in the platform or by email, and ask you to accept the new terms at sign-in.

14. Governing law and contact
